Birth and Death Records
Computer downlink allows Jasper County
Health Department access to vital records at Missouri Department of
Health in Jefferson City, Missouri.
Certified copies of Birth Certificates can be printed at our office
while you wait. Certificates are available for any birth
occurring in the State of Missouri after 1920.
Certified copies of Death Certificates can be printed for any person
whose date of death is after 1980.
If you need a Death Certificate for a Death that occurred prior to
1980, please contact the State at:

Birth Certificates are $15.00 each. Death Certificates are $13.00 for the first one, and $10.00 for each additional copy. The Health Department accepts personal checks, cash or money orders.
To request a death
certificate, print the following form, complete, and bring or mail
to the Health Department along with the money order per certificate
fee. Certificates are available immediately if you present the
form in person. Certificates requested by mail are processed
on the day of receipt.

If your Birth
or Death Certificate does not print correctly you need to
download the newest version. |
To obtain a birth or death record, you will need
- Full name
- Date of birth or death
- Hospital where birth or death occurred
- Your relationship to the individual named on the record
- The purpose for the copies
- For birth records, also include Father's name and Mother's maiden name
